Welcome Family is a program led by the Massachusetts Department of Public Health to strengthen systems of care for families and children.  Welcome Family nurses offer a universal one-time visit and follow-up phone call to all caregivers with newborns, with the goal being to serve as an entry point into the early childhood system of care for families with newborns.  The program is free to parents who have given birth to a child at Baystate Medical Center, or who are residents of Springfield but gave birth elsewhere.  Home visits are provided in-person or virtually by parent preference in Springfield, Holyoke, and many of the surrounding communities.

Job Description

Welcome Family Springfield Holyoke is looking for full or part-time Maternal Child Health Nurses to offer universal, one-time home visits to moms and newborns in Springfield, Holyoke, and the surrounding communities.  Visits are provided in-person or virtually by parent preference, up to 8 weeks following delivery.  Welcome Family partners with Baystate’s Babies at the Wesson Women & Infants’ Unit, Baystate Medical Center. 

  • Conduct up to 90 minute visits at the family's home or by telehealth
  • Lactation consulting experience a plus!
  • Provide screening, brief intervention, and education to all families, regardless of perceived risk or need.
  • Offer referrals to services for caregivers with an identified need, including MHVI-funded home visiting programs and other community resources.
  • Address the caregiver's individual questions and concerns during the visit.
  • Discuss the contents of the Welcome Family gift bag with the caregiver.
  • Conduct the assessment and present information in an unbiased, culturally appropriate manner, particularly related to sensitive topics.
  • Conduct a follow up contact with all caregivers 2-3 weeks post initial visit to document the outcomes of the referrals made during the visit to assess the need for any additional referrals.
  • Collect data using the Welcome Family forms.
  • Participate in the Welcome Family Learning Collaborative.
  • Bilingual applicants are preferred

Qualifications

  • Current licensure as an RN by the Massachusetts Board of Registration, Division of Professional Licensure, with either:
    • Bachelor's Degree in Nursing or Master of Science Degree in Nursing in Maternal and Child Health, Family Health, Community Health, or related specialty
    • Two or more years of professional work experience with prenatal, newborns, pediatrics, lactation, or maternal services
  • Proficiency in Spanish preferred
  • Lactation consulting experience a plus!
