Job Description

You will be responsible for providing high-quality nursing care to patients in the community. You have clinical skills and knowledge of wound management, infusion therapy, palliative care, and chronic disease management.

  • Provide patient-focused, safe, holistic nursing care and work collaboratively with other members of our team
  • Ensure that nursing care provided to our patients is based on current and best practices to achieve successful outcomes
  • Continually assess, anticipate, identify and respond to new and changing needs of our patients
  • Travel within your region between patients’ homes

Qualifications

If you have these experiences, skills and qualifications, you are likely a good match for our patients: 

  • Minimum of two-year nursing experience (Community Nursing experience is ideal, but not required)
  • Currently registered and in good standing with the College of Nurses of Ontario (CNO)
  • Ability to work independently in managing your own caseload
  • Proficiency in English, both verbal and written required (if you can speak additional languages, terrific)
  • Valid Ontario driver's license and reliable vehicle
  • Experience with infusion therapy, mental health, palliative care, and wound care (would be great)
  • Excellent patient assessment and teaching skills
  • Problem-solving and critical thinking skills
  • Physical ability to perform your day-to-day duties
  • Basic proficiency in Word, Excel and ability to learn software
